This app will help you fill out the claim form for your destruction or loss of property when Metro Public Works took protesters' property in garbage trucks to the dump on June 28, 2020.

There are three pages of easy questions to answer. At the end of it, you'll be able to sign the form online. Then, you can either download the form and email it later or you can email it directly to Public Works (and yourself) from within the app. If you want to include photos of your lost, destroyed, or damaged property, we recommend downloading the claim form and emailing it with the pictures. You could also email the claim form and send the pictures separately to RiskMgmtDivision@louisvilleky.gov.

On this first page we'll get info about you. On the second page, we get info about what happened and the names of a couple of witnesses. On the last page, you'll finalize the document with your signature. Then, you can download it to send later or email it now.

While Kentucky Equal Justice Center built this app, we're not your lawyers and can't help you collect this claim beyond giving you this tool to fill out the form to get compensated for the loss, destruction of, or damage to your property.
